Goals of the Armed Forces Wellness Center​

Armed Forces Wellness Centers (AFWCs) provide programs and services that improve and sustain health, performance, and readiness of the Total Force​​ delivered by highly trained health professionals. AFWCs foster healthy behavior change through evidenced-based health education / health coaching utilizing advanced fitness testing technology. The AFWC mission is centered around improving health and building readiness by targeting the behaviorally-modifiable factors most likely to result in chronic disease, injury and/or performance issues.



AFWC & Military Units
  • Increase the lethality of Service Members by targeting physical fitness and overall performance.
  • Provide programs and services that improve musculoskeletal injury risk factors by proactively supporting the lowest-performing 25% of Service Members.
  • Reduce lost and limited-duty time due to injury.

AFWC & Medical Providers
  • Provide tools to address lifestyle behaviors holistically and over time.
  • Allow for medical provider direction and oversight of patient progress through Electronic Medical Record (EMR) documentation and PCMH integration.
  • Credentialed Providers can request the AFWC for specific services through the EMR to assist their patient in behavior modification and can track progress over time.

How the AFWC Works
  • The AFWC is a DHA program that falls under the provision of the local installation's Military Treatment Facility (MTF). 
  • The AFWC is staffed by highly trained Health Educators who possess a Bachelor’s or Master’s degree in an Allied Health field (Exercise Science/Exercise Physiology preferred) from an accredited college or university, and have one or more nationally recognized NCCA credentials (i.e., ACSM, NASM, NSCA,).
  • Referrals can be made to AFWC by medical providers or unit commanders. Self-referrals are always welcome and can be made by contacting AFWC directly.


Frequently Asked Questions

Q: Do I need a referral to be seen in the Armed Forces Wellness Center?

 A: There are three ways to be seen in the Wellness Center

  • Referral by a healthcare provider
  • Referral by your unit
  • Self-referral.  Although walk-ins to schedule an appointment are welcome, you are encouraged to call your local AFWC so we can tailor your appointment to meet your needs and provide any special instructions
Q: Can Family members and Retirees be seen in the Armed Forces Wellness Center?

A: All Military Health System beneficiaries are eligible for Armed Forces Wellness Center services. This includes Active Duty Service Members and their Families, Retirees and spouses, and DoD Civilians.

Q: What if I PCS?

A:  All visits are charted in your electronic medical record and also in the Armed Forces Wellness Center web-based system so your record will follow you and you can pick up where you left off with your wellness plan.

Q: What if there is not an Armed Forces Wellness Center near me?

A: AFWCs also provide virtual classes and virtual health coaching services. Call the AFWC closest to you to learn more!

Health coaching serves as the cornerstone of the AFWC program, providing the essential guidance and support needed to integrate and sustain long-term wellness outcomes. Health coaching offers personalized support to help you set and reach your wellness goals - whether it's improving fitness, managing stress, or building healthier habits. We meet you where you are!

AFWC Core Programs

The AFWC approach to service is holistic. AFWC staff members take into account all of an individual's physical, psychological and social circumstances when providing services. A holistic approach is important because a person's health cannot be fully addressed unless we consider the whole person.

Health assessment review - An analysis of a person's health status, risk for disease, and ability to increase physical activity safely.

Physical fitness - Using advanced fitness testing technology, physical fitness level is assessed and used to create an individualized exercise prescription.

Healthy nutrition - Use of metabolic testing that synchronizes an individual's resting metabolic rate to provide tailored strategies for weight loss, gain or maintenance.

Stress management - Education in biofeedback, stress relief techniques, and positive coping skills.

General wellness education - Classes on topics such as healthy lifestyles, good sleep habits, increased resiliency, preventing chronic disease through healthy living habits, and self-care.

Tobacco education - An assessment of an individual's readiness to change with a discussion of possible options for becoming tobacco-free.​
