Request Nonionizing Radiation Services

Requests for Nonionizing Radiation (NR) Evaluations, NR Training, NR subject matter expert requests, and other requests are required to be submitted to the DHA Operations Center RFI Mailbox (see below) using a DHA Form 435 (right click, save link as). Incomplete forms will cause a delay in processing your request.
