​​​​​​​​​Requests for onsite support or help from a subject matter expert from IHFSD Industrial Hygienists or Engineers must be submitted to the DHA Operations Center RFI Mailbox (see below) using a D​HA Form 435. To fill out the form, it must be downloaded and opened in Adobe Acrobat, or a compatible application. Incomplete forms will cause a delay in processing your request. Please follow the instructions below and email to both addresses to initiate a request for assistance.  A member of the IHFSD team will reach out to the main point of contact listed on the request form. ​​

1-3: ​​M​ain po​int of contact for the request.
4: Select the D​efense Health Network you belong to. (i.e., if in National Capital Region, Select DHN NCR)
5:List your Medical Treatment Facility (MTF).  If not part of an MTF, use the closest geographical MTF.  If not listed in the dropdown, skip and go to #6.
6​:
Type in the nearest MTF.  i.e. “Kirk Army Health Clinic”​
7:If customer knows a network point of contact, add it in 7. 
8:If customer knows their MTF point of contact, add it in 8.  ​
9:Leave blank.
10:
10 a-c same as blocks 1-3 or an alternate point of contact.​
11:Short description of the request.  This will become the title of the tasker.
12:Include the text “Task to: AD-HCA-PH DCPH-A OHS. Request that the Industrial Hygiene Field Service Division (OHS), Defense Centers for Public Health - Aberdeen provide services”. Include as much request detail as possible. Add the 5 “W”s to explain the request. (Who is requesting support? What support do they need? Where is it needed? When should it happen? Why is it needed?)

​The date the work should be completed + 7 days.  (Note: DCPH-A must complete the tasker approximately a week before the final due date because it must pass through 3 levels of review and close-out above us.  Each level above us takes 1-2 days of the final suspense to do their part.)

Submit the form:

Full instructions to submit are at the top of the DHA 435 in the “Instructions" box.  Generally, the process is as follows:

  1. Form must be e-mailed to their supporting Defense Health Network.  The Network reviews the request to determine if it is a legitimate request, and if the Network  can provide the requested support. See: https://health.mil/About-MHS/Defense-Health-NetworksExternal Link
  2. If the Network is unable to support a legitimate request, they will create a tasker in ETMS2, attach the DHA 435, and route the form to the DHA Operations Center's RFI/RFA Team: ETMS2 Name: DHA-RFI RFA
  3. Upon receipt, the RFI/RFA Team will task our organization to complete the work.
