MilTICK

​​MilTIC​K is a free tick testing and identification service available to Department of War (DoW) personnel and their dependents. Ticks found biting humans are identified by species, evaluated for attachment duration and tested for pathogens known to cause illness in people. The results are reported back to the point of contact provided on the MilTICK form. ​

How ​​do I submit a tick to MilTICK?

There are two ways to submit human-biting ticks for testing. Digital submission is preferred and recommended for faster results. 

Digital ​​tick form submissions (preferred)

  1. Review the instructions for submitting digital MilTICK forms.
  2. Complete the Digital Tick Submission Form (LIDS 8519) for one or multiple tick samples. The dropdowns (Columns C, D, F, G, K, M, N and O) on the digital form are available after you assign each tick a Sample Identification number (Column B). 
  3. Email the completed form to MilTICK. You may email the completed form before sending your tick samples in the mail. You don’t need to fill out individual LIDS 850 paper forms for each sample when you use this digital form.
  4. Clearly label each tick sample with the unique Sample Identification numbers assigned in the digital form (Column B).
  5. Package all tick samples and the current year USDA permit​ and mail (expedited shipping preferred) to:

Paper tick form su​b​​missions

  1. Download and print the Printable MilTICK Kit (también disponible en español).
  2. Complete page one of the tick submission form. A valid email is required to process your tick sample.
  3. Package your tick samples, the tick submission forms and the USDA permit (included in the printable kit) and mail (expedited shipping preferred) to:

​​Who is eligible to s​​ubmit a tick?

Individuals located in the United States (CONUS) in the​ following categories are eligible to submit human-biting ticks to MilTICK:

  • Active-duty service members
  • National Guardsmen
  • Reserve component personnel
  • Civilian personnel working for the DoW and military services, including Army Corps of Engineers
  • Contractors supporting the DoW and military services
  • Retired service members
  • Dependents, including spouses, parents, or children of all above categories​
